On this page, you can find the application form for a blue card for the Czech Republic.

You can also find a link to the application by the vacancy in the register of vacancies available for blue cards.
If you use a link from the register of job vacancies, the portal will automatically fill in the job vacancy number and other details of your future employment.

If you have concluded an employment contract for a job vacancy that is not published on the portal, find out the job vacancy number from your future employer and fill in the information about your future employment in the form yourself.

Other Forms

Announcement of a foreigner of termination of employment, change of employment or change of employer
The form is submitted by a blue card holder at the Department of the Asylum and Migration Policy of the Ministry of the Interior of the Czech Republic in the case of early termination of employment, change of employment or change of employer (Section 42i of Act No. 326/1999 Sb., on the Residence of Foreigners in the Czech Republic). During the first two years staying in the territory on a blue card, it is necessary to request the Ministry of Interior of the Czech Republic to fill in the job application or the employer in advance by filling in the Blue Card application form. After two years, this notification form is sufficient.

Employer's notice of non-commencement or early termination of employment
Employer's notice of non-commencement or early termination of employment of a foreigner in the territory of the Czech Republic (Section 88 of Act No. 435/2004 Coll., on Employment).

Employer's announcement on the expected change in the employment of a foreigner
The form is submitted by the employer of a foreigner with a blue card at the workplace of the Department of Asylum and Migration Policy of the Ministry of the Interior of the Czech Republic (Section 107 of Act No. 326/1999 Coll., on the Residence of Foreigners in the Czech Republic).
